Just download the .zip or .rar file (they are archive files) from the website and save it where you want. After open it with a program called WInZip for the .zip file and WinRar for both and just extract the add-on (it could be a car, a track or a mod) in your main MM2 directory; the default one is C:\Program Files\Microsoft Games\Midtown Madness 2.
Then start the game and enjoy the downloaded add-on. :)

The extracted .ar add-on file must be in the main MM2 folder, while the downloaded .zip or .rar archive can well be placed where you want. :)

Is it in the cities list? You must play that track with a medium polygons car (too detailed cars could make it crash) and at least at a screen resolution of 1024x768 pixel.

You have to put all the .ar files. As fas as I remember that city has more that a .ar file.
Also, don't put too .ar files, or you'll get the game crash while first loading, or, if you've put too car .ar file, while choosing a car in the Vehicle Showcase screen the game could crash after selected a car.

Simply, don't put too .ar files. Keep its number in the game folder maximum of 30 .ar files, excluding the four default one of the game. Especially, don't use too mods at the same time (especially cop mods), and too cars (if they're too much, the game could crash while choosing a car).
